Queen City Frogs Waiver for Adults

I hereby agree to follow the QCRFC Frogs Charter, including all Rules and bylaws, while not holding QCRFC Frogs liable for any injury that may occur from the playing of, transporting to and helping with playing or any event associated with QCRFC Frogs.

Agreement to Participate - Release Form

I hereby assume all of the risks of participating in any/all activities with QCRFC Frogs including by way of example and not limitation, any risks that may arise from negligence or carelessness on the part of the persons or entities being released from dangerous or defective equipment or proprety owned, maintained, or controlled by QCRFC Frogs or because of their possible liability without fault.

I also understand that the nature of this activity involves certain dangers and risks and I voluntarily assume all risks of accident, illness or injury. I hereby release and forever discharge QCRFC Frogs and their respective employees, board members, contractors and volunteers from any and all liability for personal injury, illness, or property damage of any kind sustained in any manner arising from my participation in this volunteer activity. I agree to indemnify and hold harmless QCRFC Frogs and the sponsors from any and all claims, liability, loss and expense, including but not limited to damages, legal expenses and costs of defense in any manner arising from my participation in the league.

Photo and Video Release of Liability Form

I hereby authorize QCRFC Frogs to publish photographs or videos taken of me and using my likeness, for use in print, online, social media, and video-based marketing and publicity materials as well as other publications. I hereby release and hold harmless QCRFC Frogs from any reasonable expectation of privacy or confidentiality associated with the images or videos specified above.

I further acknowledge that my participation is voluntary and that I will not receive financial compensation of any type (unless a written agreement has been signed by me and the Executive Committee and stored electronically) associated with the taking or publication of these photographs or videos or participation in marketing materials or other publications. I acknowledge and agree that publication of said photos or videos confers no rights of ownership or royalties whatsoever.

I hereby release QCRFC Frogs, its contractors, its employees, its board members, and any third parties involved in the creation or publication of marketing materials from liability for any claims by me or any third party in connection with my participation.
